Notice to Holts Summit Patrons

NOTICE TO ALL WHO LIVE IN OR NEAR HOLTS SUMMIT

Beginning in August 2009, if you live in Callaway County and are in or near Holts Summit, you can receive your food pantry and commodities on the 2nd Thursday of the Month from 1-4pm at the Shiloh United Methodist Church. (Just up the road from Doolittle Trailers)
In order to receive food you must PREREGISTER with SERVE on or before the 2nd Tuesday of that month.
You can pre-register in one of the following ways:
1. Call SERVE at (573)642-6388 x 1 and give the front desk the information.
2. Fill out the form at the SERVE office.
3. Email serve2@socket.net the following information:

• Month you will be going to the Shiloh pantry
• Full Names and Ages of ALL persons in your household (including yourself)
• Address (both physical and mailing addresses if different)
• Telephone Number including area code
• Gross Monthly Income
• Whether or not you receive food stamps
• Whether or not you receive TANF
• Whether or not you receive SSI, Public Housing or Section 8
• Whether or not you receive Medicaid
• If you were or are in the military or if you are a disabled veteran

WE MUST RECEIVE YOUR PRE-REGISTRATION NO LATER THAN 5PM ON THE TUESDAY BEFORE THE DISTRIBUTION!
If you do not pre-register you will not be able to get food from the Shiloh Pantry, however you can still access the SERVE food pantry during regular pantry hours (Monday, Wednesday and Friday 9am-noon and the 2nd and 4th Tuesdays from 5pm-7pm)
